Housing Case Manager- ERRH
Job Summary: The Housing Case Manager, reporting to the Housing Program Manager, provides comprehensive case management services to families enrolled in our Enhanced Rapid Rehousing Program. This role is responsible for implementing the Housing First Model, developing individualized case management plans, connecting families with essential resources to foster self-sufficiency, supporting housing retention through early intervention to maintain housing stability, tracking referrals and expenses, completing HMIS entries and case updates, and maintaining client budgets.
The ideal candidate is compassionate and detail-oriented and excels at navigating complex systems and building relationships with diverse stakeholders. Proficiency in crisis intervention, financial coaching, and advocacy is essential, as is a commitment to an equitable, client-centered, trauma-informed approach.
